I've forgotten my Username - What do I do?

If you have forgotten your Username:

1. From My Health Online - Log in, select Need help accessing your account?:

   

2. The Account Recovery screen displays, select I have forgotten my username:

   

3. You are prompted to add your email address and tick I'm not a robot:

   

   Note - If your email address is used for more than one patient, for example, your children or someone you care for, you are now prompted to enter your First Name, Last Name and Date of Birth and then select Recover username.
4. Select Recover username.

An email is sent to your email address which contains your username.
